Annual Meeting Registration Form

Please register for the annual meeting of The John Whitmer Historical Association beginning Thursday, September 23rd through Sunday, September 26th, at the Council Bluffs Ramada (2325 Avenue N, Council Bluffs, Iowa 51501).

Conferees should make their own hotel reservations by calling 1-800-RAMADA, 1-712-256-1777, or 1-712-328-3881. Be sure to ask for the John Whitmer Historical Association rate.

Give the name of each person registering in the space below. Each person must have his / her own name tag as identification for admittance into the sessions. Name tags and program materials will be in the registration packets provided upon your arrival.

We cannot process credit cards on jwha.info, so this form generates a printable Annual Meeting Registration form which you can mail with your check to JWHA.

PLEASE NOTE: Reservations deadline is September 9, 2004. It will be necessary to have this information by that date for reservation confirmations for the caterers. After 9/9/2004 registration will cost $40 and the Presidential Banquet will cost $30.
